Impact of Changes of the 2020 Consensus Definitions of Invasive Aspergillosis on Clinical Trial Design: Unintended Consequences for Prevention Trials?
Using more stringent consensus diagnostic definitions for Invasive Aspergillosis, more than 20% of cases in hematopoietic cell transplant and hematologic malignancies patients that met earlier definitions no longer meet diagnostic criteria. This has important implications for antifungal prevention trials.
Impact of a Multiplex Polymerase Chain Reaction Panel on Duration of Empiric Antibiotic Therapy in Suspected Bacterial Meningitis
In this pre-post study, implementation of a multiplex polymerase chain reaction panel testing for agents of meningitis significantly reduced the duration of empiric antibiotic therapy, time to targeted therapy, and hospital length of stay compared with traditional microbiological testing methods.

Long COVID and Post-infective Fatigue Syndrome: A Review
Fatigue after COVID-19 is common but generally resolves over months, like other postinfective fatigue states. Post-COVID fatigue results from end-organ injury, mental health conditions, or idiopathic post-COVID fatigue. Post-COVID fatigue should be assessed with validated questionnaires, interviews, and protocolized investigations.

Conquering COVID: How Global Vaccine Inequality Risks Prolonging the Pandemic
As the U.S. and wealthier nations encounter both setbacks and successes in their COVID-19 response, developing countries face profound inequality in vaccine access. This disparity threatens to prolong the pandemic and mire impoverished countries with devastating impacts for decades to come.

Epidemiological and Clinical Characteristics of Melioidosis Caused by Gentamicin-Susceptible Burkholderia pseudomallei in Sarawak, Malaysia
We show that gentamicin-susceptible Burkholderia pseudomallei infections are common in Sarawak, Malaysian Borneo, and dominate in the high-incidence rural interior regions. However, clinical characteristics and outcomes were similar to infections caused by more typical gentamicin-resistant B pseudomallei.
Coronavirus Disease 2019 Vaccine Impact on Rates of Severe Acute Respiratory Syndrome Coronavirus 2 Cases and Postvaccination Strain Sequences Among Health Care Workers at an Urban Academic Medical Center: A Prospective Cohort Study
Cases of SARS-CoV-2 among health care workers dropped rapidly with COVID-19 vaccination. Sequencing 50 breakthrough infections (overwhelmingly in 14 days after first dose) showed no clear sign of any differences in spike protein compared with time-matched, unvaccinated control sequences.

US-Focused Conceptual Health Care Decision-Analytic Models Examining the Value of Pivmecillinam Relative to Current Standard-of-Care Agents Among Adult Patients With Uncomplicated Urinary Tract Infections due to Enterobacterales
Pivmecillinam has improved microbiologic activity against common Enterobacterales, suggesting that this agent has the potential to reduce the economic burden associated with inappropriate treatment of adult outpatients with uncomplicated urinary tract infections.

Trends of the Epidemiology of Candidemia in Switzerland: A 15-Year FUNGINOS Survey
The incidence of candidemia increased in Switzerland from 2004 to 2018. A species shift toward C glabrata was observed after 2013, now accounting for one fourth of all candidemia, concurring with increased consumption of mold-active triazoles.
Application of a Serial Antigen-Based Testing Strategy for Severe Acute Respiratory Syndrome Coronavirus 2 and Student Adherence in a University Setting: Wisconsin, October–November 2020
Over 6 weeks, two thirds of 1754 eligible on-campus students adhered to serial COVID-19 antigen testing. Of testing encounters requiring confirmatory RT-PCR testing, 28% returned. The testing strategy captured 61% of RT-PCR-positive specimens if all testing encounters had confirmatory testing.
