A 52-year-old male presented with cardiac tamponade and left atrial thrombus (Fig. 1B and C ) adherent to occluder several months after interventional patent foramen ovale closure due to stroke. Most probable site of perforation was at the dome of the left atrium (Fig. 1A). Device and thrombus were removed and surgical correction was performed successfully.
