Sir, Hereditary haemochromatosis (HH) is an inherited autosomal recessive disorder characterized by systemic iron overload. It is relevant to rheumatologists as the cause of an arthropathy that initially affects the hands, particularly the index and middle MCP joints, then later larger joints such as the knees, hips and shoulders. It is usually considered a non-inflammatory condition, but here we report two cases of HH arthropathy found to have increased Doppler signal on US, similar in appearance to that found in inflammatory arthritis. As far as we are aware, this has not been reported in the literature previously. Our report is also of particular relevance to those using Doppler US in early arthritis clinics.

A 40-year-old Caucasian male presented with a 5 year history of pain and stiffness in the index and middle MCP joints of both hands with 10 min of morning stiffness and intermittent joint swelling. There were no seronegative features and no family history of psoriasis.
