sir, Thank you for the comments [1] on our article [2]. We do take on board the points that Collins et al. [1] have made, but we think that the article was meant for more of a global audience rather than a UK-based one. In the UK, rheumatologists are under-resourced [3], with large variation in the ratio of rheumatologists to the population, and this varies around the western world with figures from one rheumatologist per 28 571 people, to per for 142 857 people [4]. A recent example can be found in Spain, where the number was estimated at one per 33 280 inhabitants [5]. This makes resources less of an issue. Our article was more a call for more collaborative work between different teams delivering care for patients with chronic pain.
