First of all, we would like to congratulate the authors for their study entitled “Spontaneous Erectile Function Recovery Among Young Men with Erectile Dysfunction Taking Tadalafil 5 mg Once a Day” (Pozzi E, Corsini C, Bertini A, Belladelli F, Raffo M, Negri F, et al. J Sex Med 2024; 21(8):671-675). In this study, 478 heterosexual, sexually active men with erectile dysfunction were included. According to their internal protocol, the authors mentioned that color Doppler duplex ultrasound (CDDU) was performed on every patient at baseline. The patients were evaluated at the third month. The authors found that approximately half of the young patients with psychogenic ED achieved medication-free erectile function recovery after discontinuing tadalafil 5 mg once a day, following a 3-month therapy period.
The authors are well-known and highly respected in the urology community. Therefore, the results of this study are important for clinical practice. There are two critical points in ED patients receiving tadalafil 5 mg treatment. The first point is determining after how long tadalafil 5 mg should be deemed insufficient. Secondly, in patients with psychogenic erectile dysfunction or erectile dysfunction potentially due to mild/moderate vascular problems, it is important to determine how long the once-daily use of tadalafil 5 mg will continue to improve erectile dysfunction after drug discontinuation. The present study attempts to address the second point.
