Objective

Priapism is considered a urological emergency and its treatment must try to avoid erectile dysfunction. The objetives were: to describe the population that developed priapism for nine years and to analyse the effect of the treatment on erectile function.

Methods

It was done a retrospective analysis of 65 cases of priapism occurred in 45 patients in a health institution between 2007 and 2016. The IBM SPSS Statistics 23.0 was used.

Results

The mean age was 47.5 years. Each patient had on average 1.93 episodes of priapism, with a mean duration time of 24.32 hours. The most common aetiology was taking an antipsychotic drug (29.8%). All cases of priapism were ischaemic. In 82.5%, the first treatment was cavernosal irrigation. In 36.8%, a distal shunt was further done, especially the Winter’s procedure (80%). In 26.3%, it was followed by a second procedure nearly after 5.9 days: a new distal Winter’s procedure (41.7%) or only cavernosal irrigation (33.3%). In 79.6%, an immediate intraoperative resolution was obtained. It was realized corporal fibrosis in 40,4%. The immediate intraoperative resolution was obtained in those that had a less duration of erection (26 vs 55.57 hours, p: 0.03). In 32,4%, erectile dysfunction was developed, especially in the elderly (52.8 vs 42.6 years, p = 0.009) and those that had a longer duration of erection (33.9 vs 17.86 hours). Shunt procedure was associated with a higher prevalence of erectile dysfunction (p = 0.013). However, a second procedure was not statistically associated with erectile dysfunction.
