Objective
Impending erosion needs to be recognized and addressed promptly, before the cylinder breaches the skin and gets infected. This is the case of a 66-year-old patient who had undergone implantation of a 3 pieces inflatable penile prosthesis in 2015 for the management of refractory erectile dysfunction following robotic prostatectomy. Distal dilatation was challenging due to corporal fibrosis and this was addressed with second, distal corporotomy and glanspexys. The patient presented with impending distal erosion of the right cylinder 1 year postoperatively. The clinical findings have been confirmed with magnetic resonance of the penis, which has shown that the tip of the right cylinder had eroded through the tunica albuginea.
Methods
Revision surgery has been carried out through a ventral subcoronal incision with partial degloving of the penis. The right cylinder tip was exposed through a longitudinal incision of the tunica albuginea and a new channel was created on the back wall of the old implant capsule. Once the cylinder has been repositioned in the newly formed channel, the tunical defect was repaired with interrupted 0-PDS sutures with the interposition of the back wall of the old capsule to guarantee extra tensile strength without the need of a windsock graft.
