Objective

An often underreported complication of RP is climacturia or urinary incontinence in relation to sexual stimulation experienced by 20-93% of RP patients at least a few times after surgery. Available interventions for the management of this condition have, to date, been underwhelming. The “miniskirt” or “Mini-Jupette” is a concept developed by R. Andrianne, in which a “Polypropylene” mesh is used to permit a compression of the urethral bulbar area with an insertion to the medial aspects of the bilateral corporotomies at the time of IPP insertion. The belief is that, as the cylinders expand on inflation, this may help limit climacturia, as well as leakage in patients with minimal incontinence (1-2 pads/day) undergoing IPP insertion for ED.

Methods

After corporotomies are made, a measurement is made between the medial aspects of the bilateral corporotomies and a polypropylene graft is fashioned according to that width. The length of the graft should be the same length as the corporotomies. The graft is then sutured to the medial aspects of both corporotomies without being under tension. The corporotomies are then closed. The rest of the procedure is as per usual IPP insertion.
