After years of being at a standstill, hypertension is on the move. With the results of the SPRINT1 and HOPE-3 trials,2,3 optimal target blood pressure values are again receiving interest and with recent trials on renal nerve ablation,4–6 interventional treatments of hypertension are back at center stage. What is normal blood pressure7 is a crucial question that has influenced both the recent US Guidelines8 and now also the long-awaited ‘2018 ESC/ESH Guidelines for the management of arterial hypertension’,9 which are presented in this issue. Among various changes, the most remarkable is the new definition of target blood pressure in patients below 65 years of age on antihypertensive drugs as 120–129 and <80 mmHg, while in the elderly, the recommendations are 130–139 mmHg in the systolic range. Thus, for the first time, a blood pressure range rather than a single upper value is recommended.
