It would not have been possible to make this Manual without the skills and extraordinary dedication over many years of Shanthi Graham. Starting as Scientific Officer to the Movement Disorder Unit in 1988, she was involved in the assessments of many of the patients in the Movement Disorder Laboratory. In 1994 she was involved with Michael Hayes in the filming of patients with various gait disorders, some of whom appear in the Manual. In 1999 she began to work on the Manual, which had originally been conceived as a collaboration with Professor David Marsden of the National Hospital, Queen Square, but following his untimely demise in 1998, the plan was changed. We have nevertheless included a few of his cases and are grateful to Philip Thompson, Niall Quinn, and Kailash Bhatia for permission to do this. Shanthi has been responsible for helping to select the cases from the video database, editing videos, incorporating the text, planning the formats, and running the technical aspects of the whole operation. She has kept us focused and has provided a critical but always constructive overview. So important has her role been that we invited her to be a coauthor but, in typical modest fashion, she declined.

Institutional access
