In all cases of death during follow-up, the cause was determined. Table 4 shows the causes of death in the study population. The most frequent cause of death was non-sudden cardiac death, specifically terminal HF, followed by non-cardiac death and SD. Patients receiving CRT-D showed a relative reduction in the prevalence of HF death in comparison with patients receiving ICD only. The 6-year cumulative incidence of HF death was 20.2 ± 8.1% in ICD-only patients and 3.5 ± 2.6% in CRT-D patients (P = 0.003). The 6-year cumulative incidence of non-cardiac death was 12.7 ± 4.2% in the ICD-only group and 17.3 ± 7.5% in the CRT-D group (P = 0.911). The 6-year cumulative incidence of SD was 3.4 ± 1.6% in the ICD-only group and 3.3 ± 2.5% in the CRT-D group (P = 0.837). Figure 3 shows the cumulative incidences of HF death, non-cardiac death, and SD in the two groups. The 6-year cumulative incidence of HF death was significantly lower in responders than in non-responders; in the ICD-only group, it was similar to that of non-responders (Figure 4A). Similarly, the 6-year cumulative incidence of non-HF death (other cardiac deaths, non-cardiac death, and SD) was significantly lower in responders than in non-responders; in the ICD-only group, it was similar to that of non-responders (Figure 4B).
