aFever or systemic includes fever, rigors, night sweats, weight loss; Cardiopulmonary includes cough, hemoptysis, shortness of breath, chest pain; Gastrointestinal includes vomiting, vomiting of blood, abdominal pain or severe abdominal pain, diarrhea, blood in stool; Neurological includes headache, photophobia, neck stiffness, convulsions, lethargy, confusion; Other includes all other symptoms.

bPresence of chronic comorbidity was defined as those reporting a history of hypertension; diabetes mellitus; cancer; HIV; or any chronic heart, lung, liver, or kidney disease.

cHealth care–associated infection was defined as hospitalization within 90 days before onset of the presenting illness, surgery within 30 days, or prosthetic joint replacement within 1 year with symptoms at the site of joint replacement.

dHypoxia was defined as peripheral oxygen saturation <92% or use of supplemental oxygen.

eAltered mentation was defined as either V, P, or U on the AVPU scale.
