Two authors (M.I. and H.A.) independently verified the International Classification of Diseases, Tenth Revision (ICD-10), codes that corresponded to each comorbidity (see Supplementary material online, Table S1), and any disagreements in inclusion or exclusion of ICD-10 codes were discussed with a third author (M.A).
