Day 0 | Patient presented as an outside hospital transfer with Streptococcus pneumoniae bacteraemia, endocarditis, meningitis, TV vegetation, and concern for implantable cardioverter defibrillator (ICD) infection. |
Day 1 | Transoesophageal echocardiogram (TEE) revealed a 3.5 mm × 11.5 mm mobile mass with independent motion on TV, as well as a 9.5 mm × 14.7 mm mobile echodensity on the ICD pacing wire, concerning for endocarditis and cardiac implantable electronic device (CIED)-related infection. |
Day 5 | Patient underwent ICD device extraction with intraoperative intracardiac echocardiography (ICE). |
Day 8 | Patient underwent right heart catheterization and AngioVac-assisted extraction of the TV mass, with near-complete removal evidenced on post-debulking TEE images. |
Day 11 | Patient was discharged to acute rehab with regular follow-up with cardiology and infectious disease. |
Day 36 | Patient completed 4 week course of intravenous (IV) antibiotics from date of AngioVac procedure per infectious disease. |
Day 99 | ICD replacement was performed 3 months following date of debulking procedure. |
