We herein report a novel ‘anterior approach’ for performing robot-assisted thoracic surgery lobectomy for lung cancer with the camera and robotic devices inserted via the anterior chest wall. The camera was placed at the 4th intercostal space (ICS) on the anterior axillary line, the first arm-port at the 6th ICS on the middle axillary line, the second arm-port at the 3rd ICS at the axilla and the third arm-port at the inferior ICS on the middle to posterior axillary line. Given its optimum view of major vessels and lymph nodes, this approach can easily be adopted by many robotic surgeons.
