The authors would like to apologise for the omission of the Improvement Study Group as a co-author of this manuscript. The data presented in this article were collected by the Improvement Study Group, represented by 25 Dutch fertility centres as part of the Dutch Consortium for Healthcare Evaluation and Research in Obstetrics and Gynaecology.
