A 49-year-old woman with Marfan syndrome underwent replacement of the aortic arch as her seventh cardiovascular surgery. By this, total repair of the aorta and the heart valves except for pulmonary valve was completed (Fig. 1 ) without any serious complications. This is a rare case even for a Marfan patient.

Reconstructed CT image shows implanted artificial valves in aortic and tricuspid position, mitral annuloplasty ring, vascular grafts in the aortic root, ascending arch and abdominal aorta and both common iliac arteries, and stent-grafts in the descending and supra-celiac abdominal aorta. All grafts are patent, and no leak or endoleak is observed.
