Linezolid Therapy: Are You Sure the Dose Is Correct?

The need for therapeutic drug monitoring (TDM) during vancomycin and aminoglycoside therapy is well recognized. Monitoring of the serum concentrations of most other antibacterials, including linezolid is rarely performed rarely in the United States. The recommended adult dose of linezolid is 600 mg every 12 hours regardless of body size or the presence of renal or hepatic insufficiency (although the product insert also lists 400 mg every 12 hours for adults—but not for adolescents!). This oxazolidinone antibiotic is a moderately lipophilic molecule that undergoes nonenzymatic oxidative metabolism, with 65% being cleared by nonrenal mechanisms and 30% excreted unchanged in the urine. There is no significant difference in bioavailability after oral or intravenous administration. I am aware of no guidelines that recommend TDM of linezolid.
