The article titled, “Safe Gluteal Fat Graft Avoiding a Vascular or Nervous Injury: An Anatomical Study in Cadavers,” addresses a critical issue in plastic surgery.1 Mortality as a result of fat embolism during gluteal fat grafting is the most pressing patient safety issue facing plastic surgeons today.2 Our plastic surgery specialty has responded with unprecedented collaboration in the formation of a multi-society taskforce including the American Society for Aesthetic Plastic Surgery, the International Society of Aesthetic Plastic Surgeons, the American Society of Plastic Surgeons, the International Federation for Adipose Therapeutics and Science, and the International Society of Plastic and Regenerative Surgery. A variety of techniques have been proposed to prevent inadvertent injury to the gluteal veins,3 but anatomic study of the gluteal region is critical to provide practical recommendations for performing the procedure. We completely agree with the importance of addressing this topic and commend the authors for rigorously studying the gluteal fat grafting procedure.
