The preceding article on calf reduction is significant for several reasons, not the least of which is to draw attention to the under reported perceptions of patients regarding the appearance of their calves.1 Patient concern regarding breast, abdominal, buttock, and facial appearance are the most commonly considered cosmetic areas of interest among lay and professional audiences. Although our clinical experience with cosmetic calf surgery has only been through calf augmentation with fat grafting it has become apparent that this anatomic area is a very significant cosmetic focal point.2

High levels of patient enthusiasm for reduction in calf contour are reported by the authors in the current report: namely a 97.5% patient satisfaction rate. The authors state that this satisfaction reporting is limited because the surgeons themselves were the source of inquiry, this high level of approval is in keeping with our own experience with patients seeking surgical calf enhancement. For patients dissatisfied with the small size of their calves, the achievement of calf enlargement along with improved balance and symmetry has been as rewarding as that seen with patients undergoing other types of body contour surgery.
