It has been brought to my attention that the language describing inadvertent intra-abdominal penetration may not be clear in my chapter “Liposuction” in Neligan’s Plastic Surgery textbook1 and in fact may be implying negligence in all cases that this occurs.

On page 526, the language implies that these complications occur due to inadequate preoperative examination. While that statement may be true in rare cases, more commonly small hernias can be missed even on thorough examination. A patient who has a larger body mass index or one with a prior history of abdominal surgery make examination even more challenging. Intra-abdominal penetration may also be related to overaggressive suctioning or even penetration of the abdominal wall by the infiltration cannula.

I am sorry if the message in the original chapter has created confusion. It certainly was not my intention to imply that any intra-abdominal injury is a result of an inadequate evaluation.
